Position summary

As a vital member of the Division of Medical Oncology and Hematology at University Health Network (UHN), the Patient Flow Coordinator (PFC) plays a crucial role in managing both internal and external patient referrals. This position is central to the new patient intake process, overseeing everything from the initial referral to the patient's first consult. Working under the Manager, Medical Oncology and Hematology, you will ensure efficient patient flow within a busy medical environment at the Princess Margaret Cancer Centre in Toronto.

Your ability to manage sensitive patient data confidentially and work effectively in a high-pressured environment will be key to your success.

  • Completion of Grade XII or recognized equivalent - required
  • Completion of Medical Office Administration Program
  • Completion of Medical Terminology Course(s) is an asset
  • Minimum of one (1) year related work experience - required
  • Previous experience working in a Health care environment - preferred
  • Previous experience working with the Electronic Patient Record (EPR) or Pathways Healthcare Scheduling (PHS) - preferred
  • Working knowledge of computer software applications preferably in a Microsoft Office environment
  • Demonstrated commitment to client service and satisfaction
  • Excellent interpersonal and listening skills
  • Strong communication, organizational, time management and problem solving skills
  • Strong judgment/decision making skills
  • Ability to remain calm and composed when dealing with difficult people or situations
  • Ability to work efficiently in a high-pressured environment with simultaneous demands
  • Adheres to confidentiality policy for sensitive patient data and information
  • Demonstrated ability to work in a team environment and collaborate with others in assisting with the delivery of services

Responsibilities

In this dynamic Patient Flow Coordinator role within the Referral Office, your primary responsibilities will revolve around ensuring a seamless intake process for patients requiring care in Medical Oncology and Hematology. You will be instrumental in coordinating appointments and managing communications for new referrals.

  • Register patients and update provider (referring, primary care) information in the electronic patient record
  • Organize referral documents and communicate with triaging physicians
  • Coordinate the scheduling of patient appointments including pulling patient clinic lists and reviewing booking calendars
  • Answering patient queries and handling busy telephone activities
  • Communicate effectively in person or by telephone with internal and external parties
  • Perform cross-functional duties, as assigned or requested
  • Perform other duties consistent with the job classification, as required.
