About the Role
The Nurse Practitioner (NP) is a Registered Nurse Extended Class professional with graduate education in nursing, possessing advanced clinical and theoretical knowledge and skill within the clinical specialty.
The NP will function in an expanded role, demonstrating a high level of autonomy and expert skill to formulate clinical decisions, manage acute and chronic illness, and promote wellness.
Trillium Health Partners (THP) is recruiting an NP who is highly skilled, agile, and adaptable with advanced critical thinking and interprofessional expertise. As a member of the interprofessional team, you will have the opportunity to advance best practice care within this new program.
This is a temporary full-time position, approximately 12 to 18 months, working Monday to Friday from 8:00 AM to 4:00 PM at the 200 Church Street site in Etobicoke.
Key Responsibilities
In this position, the Nurse Practitioner will function to full scope by conducting comprehensive health assessments, diagnosing and providing treatment, including diagnostic, pharmacological, and non-pharmacological therapies and counselling.
You will develop, implement, and document a plan of care in collaboration with physicians and the interprofessional team, providing education and follow-ups as appropriate. Additionally, the Nurse Practitioner may be involved in other clinical, research, evaluation, leadership, and collaborative activities, such as developing new standards of care and policies.
You will work collaboratively with physicians, the interprofessional health team, and patient families to provide advanced nursing practice through physical and psycho-social assessments and the development of outpatient care plans, also providing case management. You will implement care plans, assist with the evaluation of patient outcomes, and coordinate care plans with other specialists.
This role combines advanced nursing practice domains including clinical practice, research, leadership, collaboration, and consultation. You will develop effective partnerships with physicians and other members of the interprofessional team to provide quality care to patients and their families.
Applying a comprehensive theoretical knowledge base and approved medical directives, or within the scope of practice in the extended class, the NP will work collaboratively and independently to assess and manage patients’ medical care requirements across the continuum of care, demonstrating exemplary clinical assessment and decision-making skills.
You will provide a key leadership role in evaluating the quality of care and patient outcomes within the specialty population, in partnership with physicians and other members of the interprofessional team. You will identify quality initiatives that will strengthen and further develop nursing professional practice and evaluate the care provided to patients, including maintaining accurate statistics for reporting.
What You'll Bring
You must be registered with the College of Nurses of Ontario in the Registered Nurse Extended Class with a clinical focus in Adult care, and be in good standing with the College of Nurses of Ontario.
Demonstrated excellence with medically complex care practice and Complex Continuing Care for a minimum of two years is required. You should have demonstrated experience with completing comprehensive assessments on complex patients.
Experience in evaluating the quality of care and patient outcomes within the specialty population, in partnership with physicians and other members of the interprofessional team, is essential. Demonstrated effective leadership skills are also required.
Strong interpersonal communication skills and the ability to establish positive relationships with all staff, clients, families, and community agencies are crucial. Current Basic Cardiac Life Support (BCLS) certification is also required.
