Day-to-day

As a Home Support Supervisor, you will play a crucial role in leading and supporting a team of Personal Support Workers (PSWs) dedicated to providing exceptional patient care. Your daily responsibilities will involve a blend of field supervision, team development, and ensuring high standards of service delivery within the community.

  • Travel to patient’s homes in the community to oversee and support care delivery.
  • Work collaboratively with multidisciplinary teams in a community setting to ensure integrated patient care.
  • Assess patient needs according to established contractual agreements, formulate comprehensive care plans, and evaluate progress towards desired outcomes.
  • Manage staff, including conducting performance appraisals and implementing progressive disciplinary action as required.
  • Gather, record, and analyze patient and employee feedback, while monitoring key indicators such as caseload volume, staff turnover, occurrence rates, satisfaction levels, risk levels, and performance appraisal completion rates.
  • Ensure that all patient concerns and complaints are addressed and documented in a timely and effective manner.

Skills and experience

  • Current registration with the Colleges of Nurses of Ontario (CNO) in good standing.
  • Minimum three (3) years’ experience with progressive leadership responsibilities as a direct supervisor, preferably in a community setting.
  • Valid driver's license and access to a reliable vehicle for travel to patient homes.
  • Exceptional interpersonal skills and the ability to handle difficult situations in an objective, consistent format.
  • Strong ability to write reports and communicate effectively, both in writing and verbally.
  • Excellent English verbal and written communication skills (additional language skills considered an asset).
  • Solid knowledge of the principles, practices, and methods of community nursing and service delivery to Patients, as well as program development, implementation, and evaluation.
